1. A brief introductions to the purpose of the questionnaire and why you are requiring this information?
2. A minimum of 5 or 8 questions that attempt to gain an insight into the childs sense of belonging, being and becoming and therefore assist you in your future planning
What is your answer so far or What have you done so far as an attempt to solve this question?
I introduce my name , my working place name, my status and I am currently studying diploma,and I need to talk with parents few miniutes. But my teacher told i need to introduce myself link to EYLF and purpose of sheet, why I am collecting information? I need to ask the question professional way.
What are your child's greatest strengths?
Can you tell me about your child's daily routine at home?
what language does your child speak at home?
Could you tell me about your child in order to know her better?

For the questionnaire you could say something like:
